How to Register: It is now our Practice Policy to request and see proof of identity and proof of being ‘ordinarily resident in the UK’. You will be asked to produce a passport or photo card driving licence or other official document containing a photograph and a recent utility bill (within the last 6 months) containing your name and address. You may express a preference of doctor when you register with the practice. Please complete our questionnaire with your past medical history and then make an appointment to see the practice nurse for a health assessment which includes urine and blood pressure checks. You may then make an appointment with your doctor to discuss any health problems or worries you may have. If you are already on repeat medication, which needs to be transferred to our computer, please make an appointment with your doctor. To enable us to fully complete your registration details, please ensure that all forms are signed and returned to us as soon as possible.
Newborns: You will be sent a FP58 form with the baby’s birth certificate; please bring this to the surgery to register your new baby.
Repeat prescriptions: If you are on regular medication, your doctor will give you a computerised repeat prescription slip and advise you when a review of your treatment is due. Please hand in the repeat slip at the surgery AT LEAST 2 WORKING DAYS before the prescription is needed. Alternatively, post the repeat slip, enclosing an stamped addressed envelope, allowing at least 1 week. Our practice staff are NOT PERMITTED to take repeat prescription requests over the telephone as this may lead to mistakes.
On-line Prescription Requests: We have recently introduced an on-line prescription request system. To register and for full instructions please ask the receptionist for an Internet Registration Form.
Test results: Please ring your usual surgery for test results after 2pm when the switchboard is quieter.
Practice Newsletter: This is produced 2-3 times per year providing seasonal information.
Medical Students: This is a training practice for medical students. All medical students will one day be practising doctors and we feel it is an important part of their education to see patients on their own and learn from them. You will be informed if a student is in attendance and you may request for them not to be present if you prefer.
Access to Medical Records: To see your medical records, please apply in writing to your usual doctor or Mrs Barbara Green, the Practice Manager.
Violent/Abusive Patients: The practice is committed to taking all reasonable precautions to ensure the safety of its employees, and endeavours to ensure protection from physical and verbal abuse. If patients are violent or abusive towards staff they will be removed from the practice list.
Complaints Procedure: We have an in-house practice complaints procedure. If you wish to make a complaint please ask the receptionist for a copy. Alternatively you may ask to see the Practice Manager at Southlea Surgery or put your concerns to her in writing and she will co-ordinate responses to any medical or clinical concerns.
Disabled Access: There are disabled parking bays and ramps into the patients’ entrances at Southlea Surgery and at the Aldershot Centre for Health. All sites have toilets adapted for the disabled. Induction loops are fitted for the hard of hearing in the reception areas at all sites.
Confidentiality: The practice is registered under the Data Protection Act. Your medical records are held in the strictest confidence and information is passed on only with your consent, within the confines of the NHS, by law, or if in the public interest. All staff are bound by strict rules of confidentiality as part of their contract of employment. The practice has a Freedom of Information Publication. You may request information under the Act by applying in writing to the Practice Manager, Mrs Barbara Green.

Your Suggestions: We are always trying to improve our services and would appreciate any suggestions. Each surgery has a suggestion box for written comments, or alternatively contact the Practice Manager, Mrs Barbara Green.
The Bryan Crocker Fund: The Bryan Crocker Fund was founded in memory of Mr. Bryan Crocker. The purpose of the fund is to provide medical and nursing equipment to assist in the nursing care of very seriously and terminally ill patients in their homes. Our practice is indebted to Mrs. Crocker and her family. If you wish to contribute to this worthwhile fund  – a fund set up by our patients for our patients –  please send your donations, payable to the Bryan Crocker Fund, to Mrs Barbara Green the Practice Manager.
